AI Outbound Calls for Clinics: Reminders, Recall and Review Requests Done by an AI Agent
Every front desk has the same pile: reminder calls, overdue patients, review requests and insurance holds that outlast a lunch break. AI outbound calls take that pile off your team. An AI agent makes the calls and sends the texts in your clinic's name, then tells you what it did.
The work that never gets done: where AI outbound calls help
AI outbound calls take on reminders, recall, review requests and insurance holds, the work a busy front desk pushes to tomorrow.
Reminders come first. A forgotten visit is a gap in the schedule and a patient who has not been seen. Recall is next. Patients overdue for a cleaning or a follow-up rarely call to say so, and the list sits in your software until someone has a free afternoon.
Review requests follow the same pattern. The ask is easy to forget once the patient has left. Insurance sits underneath all of it: benefit and coverage breakdowns, unpaid or denied claims, and long waits on hold.
Each of these jobs is repetitive and worth doing. None of them is urgent enough to beat the patient standing at the desk.
An AI agent takes the lot. It calls or texts to remind and confirm. It reaches overdue patients and invites them to book again. It asks for reviews after a visit, calls insurers and chases claims, and gets back to people who asked about your services and did not book. Your team keeps the work that needs a person.
What an AI outbound call looks like
The agent calls in the clinic's name, states the reason, takes the patient's answer and writes the result back to the portal.
At 4 p.m. Emma's phone rings. "Hello Emma, this is Larch Street Dental. I'm calling about your cleaning tomorrow at 9 a.m. Can you still make it?" Emma says she has a meeting and asks for Thursday. The agent moves the visit and confirms the new time. The change is written back to the portal, where the front desk sees who confirmed and who moved.
Nobody on your team made that call. Nobody had to find the chart, wait for an answer or retype the change.
What patients hear and how they opt out
Patients hear your clinic's name, not ours, about a visit they have or are due for. A patient can ask to stop, and your team's rules decide who is contacted.
The call or text comes in your clinic's name, about a visit the patient has booked or is due for. Texts can also go out where patients already talk with you, including iMessage and WhatsApp. A patient who does not want more calls or texts can say so, and your team decides who is contacted.
Replies are treated like any other message that reaches your clinic. The agent answers what it can, the way Aida Inbound does. Where booking is switched on, it can move a visit. When a reply needs a person, your team gets the details.
What the rules say about AI reminder calls
Both countries have consent rules for AI reminder calls and texts. This is a summary of what we verified, and it is not legal advice.
These rules concern calls and texts to patients. Insurance calls go to insurers.
Canada. The CRTC, Canada's telecom regulator, does not apply its telemarketing rules to calls that do not sell anything, and the National Do Not Call List exempts people you already have a business relationship with. Separate rules for automatic dialing-announcing devices still cover any call that plays a pre-recorded or synthesized voice, including non-sales calls. The caller must be identified. Whether a conversational AI agent counts as one of those devices is unsettled.
Texts and emails fall under Canada's anti-spam law. It implies consent for two years after a purchase and six months after an inquiry. Whether a patient's visit or question fits either window is a question for your own adviser. No law in force in Canada requires a business to tell a caller they are speaking with an AI.
United States. The Federal Communications Commission has ruled that an AI-generated voice is an artificial voice under the Telephone Consumer Protection Act. AI reminder and recall calls therefore follow the consent rules for recorded calls.
Healthcare messages have their own conditions:
- The message goes only to the number the patient gave.
- It states the provider's name and contact details.
- It is limited to one a day and three a week.
- Opt-out requests are honoured immediately.
- The patient is not charged for it.
There are further conditions. The message must serve a purpose on the rule's list, wellness checkups being one, and must carry no marketing or billing content. Check each type of message you plan to send against the rule.
We found no final federal rule requiring a business to tell a caller they are speaking with an AI. The FCC proposed one in 2024 and the status is unresolved.
Our regulations resource gives the sources and the state and province pages. Ask your own adviser how they apply to your clinic.
What staff keep control of, and what they see afterwards
Your staff choose who is contacted, for which job and at what time, and the portal shows every outcome, including what needs a person.
The agent uses your approved information and your rules. You decide which patients it contacts, for which job and when. Afterwards you see who it contacted, what they said and what needs your team.
| Job | What Aida does | What staff decide |
|---|---|---|
| Reminders and confirmations | Calls or texts patients before their visit, asks them to confirm and tells your team who has. | Which visits, how far ahead, and call or text. |
| Recall | Reaches overdue patients and invites them to book again. | Who counts as overdue and when to reach out. |
| Review requests | Asks patients for a review after their visit, in your clinic's name. | Which patients are asked, and when. |
| Insurance calls | Calls insurers for a benefit and coverage breakdown, chases unpaid or denied claims, waits on hold, and brings back the answer and the next step. | Which patients and claims, and what happens next. |
| Follow-ups on inquiries | Gets back to people who asked about your services and did not book. | Which inquiries, and when a person takes over. |

Questions clinics ask
Will patients mind an AI agent calling them?
The call comes in your clinic's name and is about a visit they already have, or one they are overdue for. You choose the timing. A patient can ask to stop, and your team's rules decide who is contacted.
Does the AI agent call insurance companies?
Yes. It calls insurers for a patient's benefit and coverage breakdown, follows up unpaid or denied claims and waits on hold so your team does not. It records the answer and gives your team the next step. Your staff decide which patients and claims it works on.
Who decides who the AI agent contacts?
You do. You choose which patients the agent contacts, for which job and at what time. The agent follows your rules and uses the information you approved. Afterwards the portal shows who was contacted, what they said and what needs your team.
What happens when a patient replies or wants to reschedule?
The agent handles the reply the way it handles any inbound message. Where booking is switched on, it can move the visit and write the change back to the portal. If the reply needs a person, your team gets the details.
How do we know what the AI agent did?
Each outcome is recorded: who it contacted, what they said and what needs your team. You see it in the portal, so nothing depends on someone remembering to ask. Aida also records each answer and reports what it sent and what came back.
Do we need patient consent for AI reminder calls?
It depends on the country and the kind of message, and we are not lawyers. In the US, an AI voice counts as an artificial voice, so consent rules for recorded calls apply. In Canada, the rules for synthesized-voice calls and the anti-spam law for texts apply. Read our summary and ask your own adviser.
