Evidence before score
The standard score allows no missing rows and no partial normalization. If a required measure is incomplete, mark it unscored. A decision score requires all 100 points to be complete. If a row has no causal path, use the alternate qualitative evidence review with no numeric total, band, scale recommendation, or cross-project comparison.
A, B, C, U
Evidence confidence caps the performance rating. Direct local outcomes with a credible comparison can support a higher cap. Projected value, vendor analysis, or incomplete evidence cannot.
