Before you calculate a score
First prove that the project can be measured and that its safety, privacy, legal, and operating checks have passed.
A strong total cannot excuse a failed safeguard. Every required approval and control must pass before the team uses the score to make a decision.

This is a team review, not a solo quiz. Bring the people responsible for the work, finances, clinical safety, privacy, security, legal review, and frontline delivery. Each safeguard needs evidence and approval from the person qualified to own it.

Evidence before score
Do not turn partial evidence into a full score. Partial normalization is not allowed. If a required measure is missing, mark it unscored. A decision score requires all 100 points. If a measure cannot be tied to the project, use the written evidence review instead. Do not issue a total, decision band, scale recommendation, or cross-project comparison.
A, B, C, U
Evidence confidence caps the performance rating. Direct local outcomes with a credible comparison can support a higher cap. Projected value, vendor analysis, or incomplete evidence cannot.
